Please fill out sections A and B completely. The place of birth should be the city and state where your child was born. Please return the completed application along with a copy of your child's birth certificate. The application and a copy of the birth certificate can be returned to bickmeierd@cmsd.k12.pa.us or to the high school main office, Monday - Friday, 8:00 am - 2:00 pm.
For out-of-district and middle school students, once your permit is processed, you will be contacted to pick it up in the high school main office during the designated hours. When school is in session, CM high school students will be given their permits during the school day. Please allow 1 - 2 business days for processing.
